Take the stress out of preparing your child’s earthquake kit and let Friends of Beethoven handle it for you! The cost for each packet is $20.00 (one packet per child). 

To order an earthquake kit, simply submit a completed Emergency Contact Form (provided by your teacher the week of 8/31), an empty Ziploc bag (provided by your teacher the week of 8/31, and pay online or via cash or check.

To proceed with ordering, follow the steps below based on your preferred form of payment. Deadline to order is September 18th.

1. Submit Completed Emergency Contact Form. Your student’s teacher will send this form home with them the week of August 31st. Submit the completed form to the main office by 9/18. There is one form per student.

2. Submit Empty Ziploc Bag. Your student’s teacher will send an empty ziploc bag home with them the week of August 31st. Submit this empty bag along with the completed Emergency Contact Form to the main office by 9/18. There is one bag per student.

3. Submit Completed Earthquake Packet Order Form. Whether you pay online or in person, complete your submitted earthquake packet order form by downloading it HERE and turning it into the main office.

4. Pay for Earthquake Packet Online. Each earthquake packet costs $20.00. You can purchase more than one if you have multiple students at Beethoven Elementary. GO HERE TO PAY FOR YOUR EARTHQUAKE KIT.

5. Pay for Earthquake Kit In Person. Go the main office during normal office hours to pay for the earthquake kit with cash or check made payable to “Friends of Beethoven.”
